W-9 and Successorship Documentation Instructions

If the Builder Distributee (as its name appeared in the HOW Companies records) still exists and its address has not changed, please submit:

  • A W-9 for the Builder Distributee.

    If the Builder Distributee (as its name appeared in the HOW Companies records) still exists but has changed its name or address, please submit:

  • A W-9 for the Builder Distributee, showing the new address; and

  • A Change of Address Form.

    If the Builder Distributee (as its name appeared in the HOW Companies records) no longer exists, please submit, for each person asserting legal successorship to the Builder Distributee (e.g., by name change, dissolution, merger, acquisition, beneficiary status under a Will):

  • A W-9;

  • A change of address form; and

  • Proof of legal successorship (see Proof of Successorship Instructions and Form Affidavit and Instructions).
